Or does it propose that we will jump back to previous possibilities at some point with no knowledge of our older self? I can see that there would be branches of my choices and universes where my actions led to my deaths, but my consciousness is a continual stream forward.

“Quantum immortality” is magical thinking for people that aren’t comfortable with their own mortality. It’s functionally no different than belief in an afterlife.
That is not true. Quantum immortality does not mean you will live forever. It means that whenever there’s a non-zero chance of surviving something, you will do so. At one point you will die in all conceivable universes, at which point your consciousness is finally gone.
Of course statistically the odds of surviving through any given second decreases until eventually reaching a number so infinitesimally small that it is effectively zero.
This will happen to all of us if quantum immortality is real.
