I’ve seen a Twilight Zone episode like that. Woman is coming out of surgery and her entire head is wrapped in bandages, and when they take them off it reveals a beautiful human… To the horror of the other people in the room who are hideous aliens!

I’ve heard watership down described as dramatic irony cosmic horror, since it’s cosmic horror from the view of the characters, but the reader perfectly understands everything.
